Request Payment History

To request a payment history, you will need to provide the following information:

• Case/Cause Number OR Social Security Number
• Payor (person paying child support) Name
• Payee (person receiving child support) Name
• Self Addressed Stamped Envelope (if mailing in the request)

We can receive payment history requests by mail, fax, or email. Mail requests to:
